Cremation Paperwork


The new Cremation (Scotland) Regulations 2019 came into effect on 4 Apr 19.  As part of these regulations the old 2 page Form A was replaced by the seven new Form A’s listed below. 

  • Form A1 – Application for cremation of an adult or child.
  • Form A2  – Application for cremation of a stillborn baby.
  • Form A3 – Application for cremation of a pregnancy loss on or before 24 weeks – family arranged.
  • Form A4 – Application for cremation of a pregnancy loss on or before 24 weeks – individual or shared – health authority or body arranged.
  • Form A5 – Application for cremation of an adult or child – local authority arranged.
  • Form A6 – Application for cremation of body parts following a hospital arranged post mortem or whole body/body parts donated for anatomical examination
  • Form A7 – Application for cremation of body parts donated for anatomical examination or teaching pre‐Anatomy Act 1984.

All services where the cremation is to be conducted at Baldarroch must be supported by the documentation listed below.  The only exception to this is for Memorial Services (i.e. the coffin is being removed from the chapel for burial or a cremation has been held at Baldarroch or another crematorium at an earlier date).  In these cases only the Baldarroch Memorial Service Application Form is required.

  • The relevant Form A.
  • Baldarroch Chapel & Crematorium Supplementary Application Form.
  • Form 14 – Certificate Of Registration Of Death.
  • E(1) – Procurator Fiscal’s Certificate (if applicable).
  • For deaths outside Scotland other paperwork may be required, the details of which can be found on the relevant Form A.

All of the above paperwork must be with Baldarroch at least 24 working hours (i.e. between Monday to Friday) prior to the service/cremation.  This can be either delivered direct to Baldarroch by hand/post or sent via e-mail to info@baldarrochcrematorium.co.uk.  All of paperwork can be sent by e-mail and does not need to be supported by hard copies.

Click on the appropriate Form A, Supplementary Application Form or Guidance Notes or Memorial Application Form for downloadable copies.  Please be aware that there are no Guidance Notes for Forms A4, A6 and A7.  All of the forms are interactive PDF’s which allows you to use the ‘Fill and sign’ facility on Adobe Reader to complete and electronically sign all of the relevant sections on the forms should you so wish.  Hand written copies will of course also be accepted.
